Online & Paper Adoption Applications

If after viewing our adoptable animals on the websites or if you have seen them in person when visiting the Ukiah or Fort Bragg Animal Shelter and you have an interest to adopt an animal please go to the below adoption application links. 

The below link is to fill out an online adoption application.

When you complete the "Online Adoption Application" the completed application is automatically sent to an Adoption Counselor.  Once we receive the adoption application the Adoption Counselor will contact you to discuss your application.

You can also print out the adoption application from the below "Paper Adoption Application" link below, fill it out and either fax (468-3338) or email our Adoption Counselor the application to penryl@mendocinocounty.org
